This paper has 2 Sections Aand B.
Answer ALL questions in SECTION A and Choose TWO questions from Section B.

SECTION A: (30 MARKS) COMPULSORY
1.
Read the following case and use it to answer the questions that follow:

Metro a 42 year old male client is checked in at your clinic by a family member. According to the family member, lately he has gone into some sort of ?hiding? not leaving the house except on extremely rare occasions like to the shop. Neighbours say that he was seen leaving the house through the window alleging that some enemy is waiting for him at the door. He has lately been in great distress saying that the newspapers and radio are reporting about him so he?s worried for his life. It is for this reason that the family members thought it wise to take him for a check up. According to the family member, Metro has behaved strange most of his life preferring to be a loner with little or no interest on what was happening around him. He has never married and has not been spotted with any women as a friend. During his early years he always preferred to be on his own rather than join other children at play and preferred to remain at home when other family members went out visiting. Metro worked at a pharmaceutical firm and was once diagnosed with TB and chest complications and retired on medical grounds. Currently Metro does not have a job and leaves alone. He also takes drugs for ulcers.
With reference to the case of Metro above answer the following questions.
a)
Using the five criteria for defining a clinical problem, assess the degree to which Metro can be said to be suffering from a clinical problem/s. (10 marks)
b)
How would you use a clinical interview and a mental status examination to gather information to make an assessment and diagnosis of Metro?s case. (10 marks)
c)
Analyze Metro?s case from the information given above and attempt a DSM-IV multi axial classification. Justify your diagnosis for each axis. (10 marks)
