First of all, I would like to thank the Almighty for the good health that he has granted me throughout this entire period. My most sincere gratitude to the medical superintendent of the Kisii Teaching and Referral Hospital, Dr Enock Ondari and the health records and information department in charge Mr. Jared O toto and the whole fraternity for their guidance and support throughout this period. I would also like to thank my parents for their unending support, and finally the university for facilitating this practical session.

ABSTRACT
I was attached at the Kisii Teaching and Referral Hospital where I rotated in various departments, through the support of the senior HRIO I took part in various activities mandated in the departments.
My work cuts across the very important Health Records Issues; how they are dealt with, and challenges encountered and my suggestions on the remedies that the various stakeholders can adopt to the problems. I have categorically reported on how I participated in the handling of the following issues of Health Records Management importance: Health records law and policy planning , coding and indexing, health information systems( DHIS 2,MFL), medical statistics, wards, outpatient/accident and emergency department, office organization management, consultant clinics, diagnostics departments(laboratory, radiology), library services
