Endometriosis, an enigmatic disease.

This disease has a lot of mystery surrounding it, neither the cause nor its history is completely understood. The uterus consists of three layers: an outer, middle (muscle),inner layer (comprised of tissue sensitive to hormonal changes especially oestrogen). When tissue which is normally found only in the inside of the uterus, is found in the outside, an area where it shouldn't be, endometriosis is deemed to occur. While this misplaced tissue is usually still found in the pelvis, it can appear in distant places such as the chest cavity.

One theory claims that it is caused by the reflux of menstrual blood through the fallopian tubes into the pelvic cavity. But why is endometriosis found in areas not exposed to menstrual blood? One thought is that cells with the potential to develop into endometrial tissue are laid down outside the uterus during the development of the genital tract. These can develop into endometriosis during menarche the onset of menstruation, when hormonal cells come alive.

Before making a diagnosis, the doctor must take a comprehensive history. It starts with the very first period. Tenderness during a clinical examination can be a sign of it but what truly is a determinant of the disease is a laparoscopy. A trans-vaginal ultrasound can detect it if it is situated in the ovaries. Some of the signs are, pain during menstrual periods which is not similar to cramping. If a woman is struggling to conceive, this could be the reason.

This is a progressive disease therefore it would be helpful if it is diagnosed at an early stage. It may recur after surgery, therefore it should be cleared as much of it as possible in the initial operation. Laser vaporisation is often used , but it may not be enough as the disease is often inside,

and the excision of deep lumps may be required. Medicines may be of value in managing the pain but are less effective in the treatment of infertility.
