Kamau is a 24 year old single unemployed University dropout from an extremely

close knit, guilt provoking and argumentative family. He is admitted to Mathare

hospital three weeks after he painted everything in sight , black, green, red and white,

including his room, his furniture, his clothes and finally even himself. He was

responding to a persistent male voice that told him that his behaviour would somehow

solve the tribal problems in Kenya after the post election violence. Kamau has been

hospitalized for at least five previous occasions during the past three year, each time

4-6 weeks. Each hospitalization was as a result of exacerbation of his illness with

some combination of command hallucinations, persecutory delusions and being

violent. He has always responded well to treatment with neuroleptics but hates to

take the medication. Between hospitalizations he is likely to take medication

irregularly or not at all and to miss more outpatient appointments than he keeps. His

functioning between episodes is poor and seems to be getting worse with increasing

social withdrawal, poor personal hygiene and disordered thinking (he says people are

controlling his thoughts and actions) . However when on medication he is somehow

calm, better in appearance and speaks more coherently.
